1. Prescribing Responsibility

I have been asked to accept clinical responsibility for this drug for my patient. However, I feel I am not in a position to do this for the following reason(s)*:

Prescribing responsibility should stay with the specialist:

Drug is designated "DNP" in the JAPC Traffic Light Classification and is not recommended and not commissioned

Drug is designated "GREY" in the JAPC Traffic Light Classification. Patient has not demonstrated exceptionality and/or no evidence of first line treatments

Drug is designated "RED" in the JAPC Traffic Light Classification and is for specialist prescribing only.

Unlicensed drug / does / indication where use is not endorsed by the BNF or BNFc and/or where approved Shared Care Protocol does not exist and I (the prescriber) am unwilling to take clinical responsibility

Drug requires regular specialist monitoring and/or the majority of care and monitoring is to be supplied by the hospital and there is no approved shared care

2. Monitoring

Monitoring by specialist required (amber drug)

Patient not stabilised on the drug and / or is not suitable for hand over to primary care prescribing as per the shared care agreement

Indication not covered by the shared care agreement or continuation of treatment is outside of the parameters of the shared care

3. Lack of information

Lack of information / experience

Drug is not designated red (specialist prescribing only) but I (the prescriber) feel unable to accept clinical responsibility

Drug non-formulary in Derbyshire. Hospital prescribers are asked not to ask GPs to initiate / continue treatment with non-formulary medicines or formulary medicines outside of listed indication
